What to bring to your first appointment

Please bring the following items to your appointment:

  • Photo ID
  • Insurance card
  • List of current medications
  • Payment

The following documents may be filled out at home or in our office:

At your request and with your written consent, we may release your medical record to other healthcare providers. Print and complete the forms and submit them to our office. Please allow us 7 to 10 days to process your medical records requests.

FAQs

  • Please call the office at (901)-767-1136 to schedule. Appointments can be scheduled from 8:30 a.m. to 4:00 p.m. Monday through Thursday and from 8:30 a.m. to 3:30 p.m. on Friday.

  • We recognize that unexpected events and delays may arise. If this is the case, we ask that you please cancel the appointment within 24-hours before your appointment time.

  • Please arrive on time. We suggest that you come in at least 10 minutes early, and 30 minutes if you are a new patient. To make the registration process easier, all new patient forms can be downloaded, printed, and completed prior to your first appointment, or you may complete the forms once you arrive.

    Our office is located in the O’Ryan Building next to St. Francis Hospital on Park Avenue. There are three entrances to our building:

    • Through the valet doors on the eastern side of the building. Take an immediate left and continue until you reach the elevators.
    • Through the western entrance. Continue straight towards the elevators.
    • Through the south entrance across from the parking lot. Take an immediate right and continue until you reach the elevators.

    We are located on the 6th floor, in suite 630B.

    Free parking is available in the lot and in the parking garage south of the building. Free valet parking is available on the east side of the building.

  • All payments, co-payments and deductibles must be paid at the time of service. There are no exceptions. We accept cash, checks, credit and debit cards.

    We do our best to gather the correct insurance information and verify your benefits, however, all insurance companies issue a disclaimer that having benefits does not guarantee payment. You are ultimately responsible for all charges not covered by insurance. We recommend you call your insurance prior to the appointment to verify your mental health benefits and check if prior authorization is needed.
