Appointments can be scheduled from 8:30 a.m. to 4:00 p.m. Monday through Thursday and from 8:30 a.m. to 3:30 p.m. on Friday. Please call the office at (901)-767-1136 to schedule.
Appointment Cancellation Policy
We recognize that unexpected events and delays may arise. If this is the case, we ask that you please cancel the appointment within 24-hours before your appointment time.
When To Arrive
Please arrive on time. We suggest that you come in at least 10 minutes early, and 30 minutes if you are a new patient. To make the registration process easier, all new patient forms can be downloaded, printed, and completed prior to your first appointment, or you may complete the forms once you arrive.
Free parking is available in the lot and in the parking garage south of the building. For more information about the location and parking, please see the directions page. Free valet parking is available on the east side of the building.
All payments, co-payments and deductibles must be paid at the time of service. There are no exceptions.
We accept cash, checks, credit and debit cards.
We do our best to gather the correct insurance information and verify your benefits, however, all insurance companies issue a disclaimer that having benefits does not guarantee payment. You are ultimately responsible for all charges not covered by insurance. We recommend you call your insurance prior to the appointment to verify your mental health benefits and check if prior authorization is needed.
Required Documents for New Patients
Please bring the following items to your appointment:
- Photo ID
- Insurance card
- List of current medications

We may release your medical record to other healthcare providers with your written consent. Print and complete the form and submit to our office. Please allow us 7 to 10 days to process your medical records requests.
- Authorization for the release of medical records
- Authorization for the disclosure of psychotherapy notes
